We are looking forward to having you as a student-athlete at Francis Marion University for the 2017-2018 academic year. The following forms must be completed in their entirety in dark blue or black ink; the statement of insurance understanding form, emergency contact form, authorization for release of student-athlete health information form, shared responsibility acknowledgement form, medical questionnaire forms, concussion policy form, ADHD form (if applicable), and the sickle-cell anemia policy form (for new incoming athletes NOT returners). All these completed forms should be e-mailed to FMUsportsmedicine@fmarion.edu OR mailed (hard copies) to Francis Marion University Athletics, Athletic Training Staff, PO Box 100547, Florence, SC 29502-0547. PLEASE DO NOT FAX THESE FORMS. No student will be allowed to participate in FMU athletic activities without these forms completed on file.
Drug Testing Resources
During your time as a student-athlete at Francis Marion University you may be selected for drug testing through the institution or the NCAA. Every student-athlete and student-support staff member is subject to the department of athletics substance abuse policy from the first day of classes following their initial enrollment at FMU to their final day of enrollment. This includes summer months in-between academic years. More information on Francis Marion University’s policies on drug testing can be found in the FMU Student-Athlete Handbook. NCAA Banned Drugs and Medical Exceptions Policy (ADHD Policy)
The NCAA bans classes of drugs because they can harm student-athletes and can create an unfair advantage in competition. Some legitimate medications contain NCAA banned substances, and student-athletes may need to use these medicines to support their academics and their general health. The NCAA has a procedure to review and approve legitimate use of medications that contain NCAA banned substances through a Medical Exceptions Procedure. Supplementation and Athletic Nutrition
Informed-Choice is a quality assurance program for sports nutrition products, suppliers to the sports nutrition industry, and supplement manufacturing facilities. It is a monitoring program that certifies that a supplement product and/or raw material that bears the Informed-Choice logo has been ‘skip-lot’ tested (tested at least on a monthly basis) for banned substances by LGC’s world-class sports anti-doping lab. Supplement users can use the search function to find products that have been through this certification process. The Informed-Choice certification is a voluntary program for reputable supplement companies who wish to register some, or all, of their products and carry the Informed-Choice mark on those products. Athletes who see the Informed-Choice logo on a product can be assured it has undergone rigorous checks and testing to ensure that it is not contaminated with banned substances and is safer to use.

The Francis Marion University Athletic Department wants to assure you that our qualified athletic training staff will do everything possible to keep you or your children free from injuries while competing in our programs.
